In anticipation of Toitū Takata – LTC Programme being available in early 2024, WellSouth is offering Practice Nurses Workshop. Feedback from practices identified having Nurse Champions within the practice as a key facilitator of successfully delivering the new LTC programme within the practice. The role of the Nurse champion is not to “do everything” relating to Toitū Takata – LTC Programme, but rather empower and support others within the practice. Description

The Nurse Champions Workshops will include information on the format of Toitū Takata, engaging/enrolling patients into the programme and on providing support for those covered by Toitū Takata including the former refugee community and those with severe and enduring mental illness. This is a great networking opportunity so it is hoped that that nurses will attend face to face however, we realise that may not be possible so virtual options will also be available.

 

We will provide lunch.
