Sessions will be a total of 6 hours Cardiac Education plus 2 hours of pre-course reading. Course delivered jointly by secondary and community based cardiac nurses
Aim: To improve the Follow Up and Self-Management of people with ACS and Heart Failure in the community setting; standardising access and level of care around the Southern region. (Linking in with “Do the Right Thing” – a new approach to managing Long Term Conditions.)
Objectives: To understand ACS and Heart Failure
To utilise Best Practice Management, both of symptoms (by health care professionals and patient self-management) and pharmaceutical management, including the use of Southern Healthpathways.
To develop individual action plans and goal setting; to develop portals/templates for structured review of people with ACS and/or Heart Failure
To enhance the seamless transition with integrated care between hospital and community settings. (Using a patient case study approach, from acute onset and secondary care to long term management and community care.)
